Program Description:

The Seniors Community Connector program is under the umbrella of Golden Community Resources Society (GCRS).  Coordination and effective collaboration amongst our community-based senior’s wellness supports will optimize the quality of life for older adults. This is an exciting new program serving the community of Golden and our rural communities in Area A.  

Job Description:

Our Seniors Community Connector will integrate ‘Social Prescribing’ – a community-centred approach to mobilize the power of a community to generate good health. A collaborative assessment and referral system will connect local seniors with relevant community sector organizations and services – including but not limited to physical and social activities, professional health services, paramedical/therapeutic services, mental health services, meal delivery and nutritional support, transportation, family and caregiver support. 

Key Duties and Responsibilities:

  • Create a streamlined referral system with community partners, health care professionals and other services that match senior’s unique needs.
  • Build community partnerships through collaboration, education, and understanding of isolation, poverty, wellness and health concerns of older adults.
  • Meet with community partners to develop integrated plans that enhance the lives of older adults.
  • Maintain client database.

Qualifications/Requirements:

  • Bachelor’s degree in social service or related Human Services field or a combination of education and work experience.
  • An appreciation of how social prescribing can lead to a range of positive health and well-being outcomes and generate social value. 
  • Experience working with seniors and diverse populations and the ability to work with people from different cultures and socio-economic backgrounds.
  • Excellent organizational and time management skills.
  • Knowledge of community resources, programs and services.
  • Experience in recruiting and supervising volunteers.
  • Strong computer skills – Microsoft Office including Excel and social media
  • Valid driver’s license and reliable vehicle.
  • Reliable, consistent, mature and respectful.

Compensation

$29.00-$32.00/per hour
